هراس اجتماعی

هراس اجتماعی  در کودکان، شایع ترین اختلال اضطرابی است و شواهدی وجود دارد که نشان میدهند شیوع این اختلال در حال افزایش است . ۱۳٪ آدمها در مرحله ای از زندگی خود دچار هراس اجتماعی می شوند .
عده ای معتقدند هراس اجتماعی بسیاری از کودکان مبتلا به این اختلال آن قدر تشخیص داده نمی شود تا کار کودک به امتناع از مدرسه رفتن می کشد.
این اختلال نسبتا زود شروع می شود تا جایی که حتی بعضی از ۸ ساله ها دچار این اختلال تشخیص داده میشوند .
وانگهی اکثر کودکان مبتلا به هراس اجتماعی یادشان نمی آید در کدام مقطع از زندگی خود، اضطراب اجتماعی نداشته اند.

ویژگی اصلی هراس اجتماعی:

ترس شدید و نامعقول از قرار گرفتن در وضعیت های اجتماعی یا عمل در حضور کسانی است که احتمال دارد او را زیر نظر گرفته باشند.

شخص مبتلا به هراس اجتماعی از این که کاری کند که او را مسخره کنند یا شرمسار شود می ترسد.

 کودکان مبتلا به هراس اجتماعی از ورود به محافلی که مایه نگرانی آنها هستند مضطرب می شوند.

وقتی در چنین محافلی حضور دارند مضطرب می شوند و تا حد امکان از این محافل پرهیز می کنند.

کودکان مبتلا به هراس اجتماعی، ترس خود را افراطی و ناموجه نمی دانند. 

هراس اجتماعی می تواند از نوع «تعمیم یافته» (یعنی شخص از اکثر موقعیت های اجتماعی، اعم از عمومی و میان فردی، بترسد).

یا از نوع اختصاصی باشد (یعنی شخص از وضعیت خاصی مثلا از مهمانی ها یا حضور در باشگاه بترسد).

وقتی نشانه ها حداقل ۶ ماه ادامه یابند، تشخیص هراس اجتماعی را پیش می کشند.

متوسط سن شروع برای کودکانی که بابت هراس اجتماعی به درمانگاه ارجاع داده میشوند از ۱۱ سال تا ۱۲ سال متغیر است .

بایدل، ترنر و موریس (۱۹۹۹) در مطالعه ای، ۵۰ کودک مبتلا به هراس اجتماعی را با ۲۲ کودک بهنجار گواه مقایسه کردند.
آنها فهمیدند کودکان مبتلا به هراس اجتماعی خیلی ناراحت بودند.
آنها در امور اجتماعی، درسی و خانوادگی خود مشکل داشتند.
ولی رفتارهای آنها به شکلی نبود که پدر و مادر یا معلمان مجبور شوند از متخصص کمک بگیرند.
این بچه ها، دوستان کمی داشتند، به شدت تنها بودند و از کارهای فوق برنامه طفره می رفتند.
آنها به طور کلی خیلی اضطراب داشتند. یک رشته ترس های خاص نسبتا عذاب آور هم داشتند.
شکایات جسمی سردرد و دل درد هم در میان آنها رایج بود.
موردهای شدید هم از مدرسه رفتن امتناع می کردند و خموشی انتخابی داشتند.

این بچه ها مهارت های اجتماعی کافی نداشتند.

دلیلش هم این بود که از ارتباط با همسالان اجتناب می کردند.
انزوای اجتماعی اولیه این کودکان میتواند آنان را از فراگیری مهارت های اجتماعی لازم بازدارد و سبک منفی/ اجتنابی آنها را در کنار آمدن تشدید کند.
کودکان مبتلا به هراس اجتماعی در معرض ابتلا به خطر سایر اختلالات بودند.
۳۶% نمونه آنها نیز اختلال اضطرابی دیگری عمدتا اختلال اضطراب جدایی، داشتند.
هراس اجتماعی و اختلال اضطراب جدایی ظاهرا در دو گروه بالینی و اجتماع، خصوصا بین کودکان ۸تا ۱۲ ساله وجود دارد؛
ضمن این که نشانه های هراس اجتماعی در بین نوجوانان از نشانه های اختلال اضطراب جدایی بیشتر است .
هراس اجتماعی محصول مشترک آسیب پذیری های زیست شناختی و روان شناختی و استرس ها و ضربه های زندگی است.
 خودداری رفتاری  می تواند پیش درآمد هراس اجتماعی باشد .
وقایع محیطی دخیل در هراس اجتماعی عبارتند از ایرادگیری و کنترل شدید والدین، طردشدن از طرف همسالان .
تداوم هراس اجتماعی به چرخه افکار، احساسات و رفتارهای منفی خصوصا به نداشتن برخی مهارت ها و اشکالات عملکردی هم برمی گردد.

هراس خاص

ویژگی اصلی هراس خاص ترس بسیار شدید و مفرط از یک شی یا رویداد خاص یا ترس مفرط از پیش بینی مواجه شدن با آن شی یا رویداد خاص است.

این ترس ها مداوم و ناسازگارانه اند، به اجتناب از شی ترسناک منجر شده و باعث می شوند کودک ترس خود را غیر قابل کنترل بداند.
چنین ترسی اغراق آمیز است ولی از نظر کودکان افراطی و ناموجه نیست.
تشخیص هراس خاص وقتی مطرح می شود که:
حداقل ۶ ماه طول بکشد.
از ویژگیهای مقطع سنی کودک نباشد.
 برای کودک مزاحمت ایجاد کند.
ترس بعضی از کودکان با توجه به مقطع سنی آنها، ترس بجایی است.
ولی هراس چنان شدید است که برای کودک مزاحمت ایجاد می کند و بنابراین باید درمان شود.
هراس ها در سنین پایین هم پیش می آیند، در کودکان و نوجوانان مشهودند و الزاما معلول ضربه های تسریع کننده نیستند .

هراس های شایع دوران کودکی عبارتند از هراس از حیوانات و هراس از طوفان، تاریکی، سوزن سرنگ و بلندی.

میزان ترس کودک به میزان نزدیک بودن محرک با وضعیت ترسناک و امکان گریز بستگی دارد.

محور ترس هم صدمه ای است که کودک حدس می زند از فلان جنبه شی یا وضعیت ترسناک خواهد خورد.

امتناع از مدرسه رفتن

یکی از نشانه های شایع کودکان ترسو و مضطرب، امتناع از مدرسه رفتن است .
هر چند در ویراست چهارم راهنمای تشخیصی و آماری اختلالات روانی به آن اشاره نشده است.
هم اکنون برای توصیف این کودکان از اصطلاح «امتناع از مدرسه » استفاده میشود.

امتناع از مدرسه رفتن طیفی دارد:

که یک سر آن گاهی مدرسه نرفتن است و سر دیگر آن به زور مدرسه رفتن یا برای همیشه مدرسه نرفتن.
امتناع از مدرسه رفتن به طور مشخص در مورد کودکان ۵ ساله تا نوجوانان ۱۷ ساله ای بکار می رود که:
(۱) از مدرسه رفتن به طور کامل امتناع می ورزند؛
(۲) صبح به مدرسه می روند ولی در ساعات درسی، مدرسه را ترک می کنند؛
(۳) مدرسه می روند ولی صبح کلی قشقرق راه می اندازند و بعد مدرسه می روند .
(۴) از حضور در مدرسه خیلی ناراحت هستند و یکسره التماس می کنند که دیگر مدرسه نروند .
امتناع از مدرسه رفتن به این دلیل مهم است که تحصیل طبق قانون، اجباری است.

 مدرسه نرفتن تاثیرات بدی بر کودک و خانواده می گذارد.

اگرچه کودکان در هر مقطعی می توانند از مدرسه رفتن امتناع ورزند ولی وقتی می خواهند به مدرسه جدیدی بروند، مشکل آنان اوج می گیرد.
میزان شیوع امتناع از مدرسه رفتن در بچه مدرسه ای ها ۱ تا ۱۰ درصد و در بین پسرها و دخترها برابر است .
 مشکل امتناع از مدرسه رفتن به این چهار دلیل برمی گردد:
(۱) اجتناب از وضعیت ها با فعالیت هایی که عاطفه منفی (مثلا اضطراب، ترس، افسردگی) ایجاد می کنند.
(۲) گریز از وضعیت های اجتماعی یا وضعیت هایی که در آنها ارزیابی های آزارنده صورت می گیرد.
(۳) جلب توجه ( برای مثال کسب منافع ثانویه اضطراب جدایی) .
(۴) تقویت های محسوس (مثلا پرسه زدن با دوستان، کارکردن یا دیر از خواب بیدار شدن). 
گاهی اوقات امتناع کودک از مدرسه رفتن به دو یا چند مورد از این دلایل برمی گردد.
این رویکرد، زمينه ابداع روشهای درمانی خاص برای هر کودک را فراهم آورده است .
اضطراب مدرسه رفتن ظاهرا به مشکل جدایی یا ترس شدید از برخی جنبه های مدرسه بر می گردد .
کودکان مبتلا به مدرسه هراسی بیش از کودکان دچار اختلال اضطراب جدایی احتمال دارد از مدرسه اجتناب ورزند .

شواهد دیگر نشان می دهند :

بر اساس اختلالات اضطرابی والدین می توانیم شیوع اختلالات اضطرابی کودکی را که از مدرسه رفتن اجتناب می ورزند، پیش بینی کنیم :
والدین بچه هایی که اجتناب از مدرسه ناشی از هراس دارند معمولا هراس خاص یا اجتماعی دارند.
اما والدین بچه هایی که اجتناب از مدرسه آنها معلول اختلال اضطراب جدایی است، بیشتر اختلال وحشتزدگی یا هراس از مکان های باز دارند.
