مراحل رشد تکلم
در بین نظر روانشناسان مختلف مراحل رشد به صورت های مختلفی مے باشد و در حدود و زمان آنها اتفاق نظر ندارند ولی به طور خلاصه می توان مراحل ۵ گانه ای را که عمومیت بیشتری دارند را به این شرح بر شمرد.
مرحله اول: آغاز تولد کودک در همان بدو تولد با فریاد و گریه همراه است فریاد و گریه های نخستین که کودک در دوره پس از تولد خود بروز می دهد به علت تغییر نحوه تنفس کودک در دوره بعد از تولد با زمانی که در رحم مادر بوده می باشد و این فریاد باعث می شود که در کودک نظام تنفسی تازه ای به راه بیافتد و در ماههای دوم و سوم فریاد ها و گریه های کودک شکل و فرم خاصی پیدا می کنند. در تحقیقاتی که روی آهنگ گریه کودک به عمل آمده است نشان میدهد که گریه کودک در ماه اول بی شکلیش را از دست می دهد و برای هدفهای متفاوتی گریه زیر و بم های خاص کودک را داراست. که در هنگام تولد به اولین بازدم معروف است که در نتیجه تنفس هوای بیرونی از رحم مادر می باشد که تا به حال این تجربه را نداشته است و با گریه کردن این هوای تنفس شده را خارج می کند). در حدود ۳ ماهگی کودک متوجه صدای هایی که خودش تولید می کند است و از بازی با آن صداها شاد شده و لذت می برد، کودک نسبت به صداهای والدین حساس میشود و به آنها گوش میدهد و در همین رابطه صدا هایی را تولید می کند که وابسته به زبان خاصی نمی باشند و نشان دهنده ی این است که کودک قادر است هر زبانی که در معرض آن قرار داشته است را یاد بگیرد…
 مرحله دوم: در این مرحله که از حدود 4،5 ماهگی شروع می شود. صداهای کودک کم کم به صورت وسیله ای برای انعکاس حالات انفعالی و میلها در او در می آید. در این مرحله کودک کم کم با احساس هر گونه ناراحتی، فریاد ها و گریه های آگاهانه خود و امیال خود را بروز دهيد و الفاظی را به کار می برد که می شنود و آن الفاظ را مرتبا تکرار می کند. در تحقیقاتی که دو روانشناس ” پری یر” و ” لویس” این نتیجه به دست آمد که صداهای اولیه ی کودک ، به حروف متحرک مربوط بوده و حروف ساکن پس از انقباض دستگاه تکلم محدودتر شده، ظاهر می شوند که این مثلا خود به رشد بدنی کودک بستگی دارد.
مرحله سوم: با رشد تدریجی کودک برخی صداهایی را که می شنود مورد تقلید قرار می دهد. برای تقلید در ست کودک مجبور است به اصواتی که می شنود گوش داده تا بتواند آن اصوات را تقلید کند، اساس تقلید در نه ماهگی قابل تشخیص است. کودک آنچه را که می شنود تقلید می کند و سعی در تکرار آن کلمات در همان موقعی که لازم است را دارد یعنی کودک بعضی از کلمات را به موقعیت های معینی ارتباط دارد و سعی دارد در همان موقعیت آنها را تکرار کند. تمامی کودکان به صورتی قادرند واژه های (آ، ا) را تولید کنند و کم کم قادر به تولید واژه های (ب، د، م) نیز می شوند، در این مرحله کودک به نام خود واکنش نشان می دهند و معنی کلماتی چون «دست نزن، نکن و …» را می فهمد و نسبت به آن واکنش نشان میدهد؛ و این نشانه آن است که به زودی کودک قادر خواهد بود که اولین واژه ها را بیان کند.
مرحله چهارم: این مرحله، مرحله ی یادگیری و مهارت در سخن گفتن و درک و فهم کلمات است. در این مرحله که از ۱ سالگی شروع می شود و تا حدود ۱۸ ماهگی به طول می انجامد، کودک کم کم قادر است مفهوم کلمات و موقعیت را درک کند و به مرور زمان و با کمک والدین نام اشياء و مسائل مورد توجه را یاد بگیرد و به مرور زمان این کلمات را به جا استفاده کند. این کلمات هر چند مبین احتياج، میل و یا انجام دادن دستوری است و هر چند که این کلمات نواقص دستوری دارد اما منظور او را می رساند و این نواقص به مرور زمان رفع می گردد.
مرحله پنجم: این مرحله که به مرحله ی تکلم معروف است.کودک قادر است ضمایر شخصی مانند من، تو، ما را به کار ببرد و به نکات دستوری دقیقتر میشود. تصاویر ذهنی کودک ارتقا پیدا می کند و در خود اشیاء یا تصاویر ذهنی خود بهره گرفته و با آنها در ذهن خود بازی کند که در این مرحله است که کودک قادر است صحبت کند و با دیگران ارتباط برقرار کند. هر چند که عواملی که توضیح داده شد در تکلم کودک نقش بسیاری دارند مانند بهداشت، محیط و … که باید به تمامی این عوامل توجه ویژه داشت که بود یا نبود هر یک از آنها در روند تکلم کودک نقش بسیار با اهمیتی دارند.
اختلالات تکلم
در تکلم، برخی از کودکان دچار اختلالاتی می شوند که این اختلالات بسیار مهم می باشند زیرا در زندگی حال و آینده ی آنها تاثیرات زیادی خواهد داشت. لازم است که والدین با این اختلالات آشنایی داشته باشند که می توانند در جهت درمان با جلوگیری از پیشرفت آنها دخالت داشته باشند، به طور کلی اختلالات به دو دسته تقسیم می شوند:
1)اشتباهات سخن گویی.

۲) معایب سخن گویی.

اشتباهات سخن گویی
تکرار یک کلمه به صورت اشتباه توسط کودک و عدم توجه والدین و نزدیکان کودک به راهنمایی برای تلفظ صحیح، علت مشخص می شود تا بعدا این کودک دچار تكلم غلط و ادای اشتباه آن کلمه با لفظ شود و یا حتی برای آنکه آن کلمه را اشتباه تلفظ نکند آن کلمه را حذف کند. به فرض مثال : چنانچه کودکی «س» را «ش» تلفظ کند و به جای دوست» بگوید «دوشت» باید والدین بلافاصله با تکرار و حوصله زیاد سعی در ادای صحیح کلمه را داشته باشند چه بسا این کوتاهی و غفلت باعث شود کودک دچار سرکشی و نداشتن اعتماد به نفس کافی در آینده شود.
معایب سخن گویی
معایب سخن گویی اثرات بسیار عمیقی در آینده کودک دارد. چه بسا سازگاری کودک را با محیط اجتماعی به خطر بیندازد و دشواریهای زیادی را در طول زندگی او به وجود آورد، سخن گویی معایب زیادی دارد که در اینجا به مهمترین آنها اشاره میشود که عبارتند از:
۱- خاموشی و تاخیر در سخن گویی
۲ نقص در شنوایی
۳- لکنت زبان
١- خاموشی و تاخیر در سخن گویی
این اختلال به علت های گوناگون از قبیل فلج عضلانی، کری، کم هوشی، اختلالات دستگاه تكلم، ضایعات و امراض مغزی بروز می کند و بر اساس استاندارد های جهانی اگر مدت آن بیشتر از ۴۲ ماه به طول بیانجامد غیر عادی محسوب می شود و در جهت کشف علت و همچنین درمان آن باید اقدام کرد و برای رفع این گونه اختلال های متخصص با استفاده از روش خاص «گفتار درمانی» به رفع معایب می پردازد.
2- نقص در شنوایی
شنوایی تنها راهی است که به وسیله آن می توان حرکات عضلات و مفاصل تکلمی را هدایت کرد. صداهای مورد احتیاج را ایجاد و از صحت آنها اطمینان حاصل کرد. هر چند که شنوایی به تنهایی برای تکلم کافی نمی باشد و احتمال دارد اختلالات شنوایی با اختلالات تکلم رابطه داشته باشد و در این زمینه فقط وسائل دقیق علمی به متخصص کمک می کند که به تشخیص درست دست یافته و به درمان موثر بپردازد.
3- لکنت زبان
اختلال لکنت زبان بسیار پیچیده و مفصل است. سعی کرده ایم در اینجا به این مبحث حساس بپردازیم. شیوع این بیماری بسیار زیاد است و در موارد مختلف موجب بروز اختلالات دیگری نیز می شود؛ و در اکثر مواقع عواملی چون، ترس، وحشت، عقده ها، اختلالات روانی، عدم اعتماد به نفس و … مربوط می باشد.
در مورد لکنت زبان تعاریف گوناگونی بیان شده است که به ذکر تعدادی از آنها می پردازیم:
 – لکنت زبان یک اختلال مربوط به رشد تکلم است که با تکرارهای مکرر و تطويل اصوات و سیلاب ها که به طور قابل توجهی سلاست کلام را مختل می سازد، مشخص می شود. درنگها و سکوت های جریان موزون تکلم را در هم می ریزد. ( کاپلان و سادوک)
– ریتم غیر ارادی گفتار، با تکرار و تردید در شروع حرف یا سیلاب ها و با کلمات یک سیلابی که موجب توجه دیگران شده و در ارتباط با دیگران باعث ناراحتی شخص شنونده گان می شود. (بلاد اشتاین)
شروع لکنت زبان در حدود ۳ یا ۴ سالگی است یعنی دورهای که در آن اكتساب شدید گفتاری جریان دارد و لکنت زبان به صورت فیزیولوژیک اتفاق می افتد. در این سن اصولا کودک در مقابل دیگران به لکنت دچار می شود اما به هنگام صحبت برای خانواده یا دوستان و همسالان خود بدون لکنت صحبت می کند. همچنین ممکن است برای کودکی که اولین بار به مدرسه می رود به علل مختلف از جمله ترس از معلم، اضطراب صحبت کردن در مقابل شنونده ناآشنا دچار لکنت گردد و اگر با این کودک رفتار صحیح و درست صورت گیرد چه بسا لکنت سریع رفع شود و اگر رفتار صحیح نشود شاید دچار لكنت گردد. بنابراین سازگاری کودک با محیط رابطه مستقیم با لکنت دارد.
انواع لکنت زبان
1) لکنت زبان کلونیک
۲) لکنت زبان تونیک
۳) لکنت زبان کلونو-تونیک
١- لکنت زبان کلوئیک
 عبارت است از تکرار تشنج آمیز یک واج یا هجا قبل از اینکه کلمه یا جمله ادا شود و یا اینکه ادامه یابد. برای مثال «مامامامان آمد» .
۲- لکنت زبان تونیک
 عبارت است از حالتی که در آن عظلات گویایی چند ثانیه تا ۱ دقیقه از حرکت باز می ایستند و مانع تکلم می شوند، و به عبارت دیگر زبان بند می آید و هنگامی که انقباض رفع می شود، کلام خیلی سریع و با شتاب ادا میشود. 
٣- لکنت زبان کلونو-تونیک
ترکیبی است از لکنت زبان کلونیک و تونیک و بر اثر برخورد نادرست و خشن با فرد مبتلا به لکنت و یا توقع بیش از ظرفیت داشتن از او موجب می شود که وی دچار ترکیبی از کلونیک و تونیک شود.
درمان لکنت زبان
1-  ایجاد اعتماد به نفس در کودک، یکی از موثر ترین عوامل در کاهش لکنت زبان است. موقع سخن گفتن کودک هر چند هم بی ربط و بی محتوا باشد به دقت به حرفهایش توجه و دقت کنیم و به شخصیت او احترام بگذاریم و به کودک بیاموزیم که آهسته صحبت کند و نیازی به کشش عظلات خود در هنگام صحبت کردن نمی باشد و آزادانه از دستگاه صوتی خود بهره ببرد.
2- در هنگامی که کودک دیر شروع به صحبت می کند نباید نا امید شد و نباید با استفاده از زور کودک را وادار به تمرین کنیم که چون این رفتار باعث ناامیدی و اضطراب در کودک میشوند و باید با تشویق کردن و باصبر و حوصله و ادای صحیح کلمات کودک را آموزش دهیم و با حوصله و پشتکار به کودک امید بدهیم تا دچار اضطراب زبان نشود.
3- کودک باید در محیط خانه احساس آرامش کند و خانه عاری از دعوا و مشاجره در خانواده باشد که تمامی این دعواها و مشاجرات باعث بروز اضطراب در کودک می شود و دچار لکنت زبان و یا اگر سعی در درمان کودک دارید باعث تاخیر می شود پس محیط امن و آرام در رشد صحيح کودک موثر است.
4- تمرین به صورت صحیح با کودک در هنگامیکه زبان باز می کند موثر است و یکی از این تمرین ها که میتوان انجام داد استفاده از همسالان و بازی کردن جمعی کودک باشد که در رشد تکلم او موثر است.
5- اگر کودکی دارای لکنت است و مطمئن هستید که وراثتی نیست و یا اگر هم وراثتی است در طول روز با او به تمرین بپردازید و تمرین مکالمات را با جملات ساده که تقلید آنها برای کودک ساده است انجام دهید و سعی کنید که کودک را در این مکالمات دچار خستگی نکنید که حتی اگر موجب بهبود کامل او نشود از رشد لکنت پیشگیری می کند.
6- اگر در خانواده کسی هست که دچار لکنت زبان می باشد، هرگز کودک را در سنی که زبان باز می کند نباید در مدت طولانی کودک را با او تنها گذاشت زیرا کودک بر اثر تقلید و تمرين لكنت را از او یاد گرفته و به آن عادت می کند.
7- حالات هیجانی کودک را تا جایی که امکان دارد کنترل کنید، زیرا هیجانات بیش از اندازه در ایجاد لکنت زبان تاثیر دارد، حتی بعضی از بزرگسالان در هنگام داشتن هیجان شدید، دچار لکنت زبان و تکلم غیر طبیعی می شوند.
8- سعی کنید با تشویق و تائید، کودکان را به تحرک و تفکر واداريد، تحرکات و تفکرات کودک زمینه ساز پیشرفت زبان و تکلم هستند و در این مرحله کودکان اصولا پرخاشگر و عصیان گر هستند، باید با حوصله و صبر آنها را هدایت کرد تا جایی که این حالات را به کنترل خود در آورند.
9- از تحقیر کردن کودکان بپرهیزید چون تحقیر و سرزنش کردن باعث ایجاد عقده حقارت می شود و از شکوفایی و بروز استعداد خوب سخن گفتن جلوگیری می کند.
10- هرگز سعی نکنید کودکی که با دست چپ می نویسد را وادار کنید که با دست راست بنویسد چون میان قسمتی از مغز که کنترل کارهای دست و را به عهده دارد و قسمت دیگری از مغز که کنترل و و سازمان دهنده سخن گفتن می باشد ارتباطی نزدیکی است و احتمال اینکه این اختلال موجب بی نظمی و اختلالات دیگری بشود است که یکی از آنها لکنت زبان است.
11- عوامل زیادی در بروز لکنت زبان موثر هستند از جمله، ترس، وحشت، اضطراب، دلهره و هیجانهای ناشی از آنها، پس هرگز کودکان را نترسانید بالاخص در سنین اولیه که کودک به آرامش و امنیت خاطر بیشتری نیازمند است.
12- دوست بودن با کودک و به کودک آرامش دادن موجب می شود که آنان با تفکر سازمان فکری خود را قوی کنند و تکلمشان به صورت بهتری رشد کند و اگر به کودک در هنگام بازی و مسائلی که مربوط به خودش است آزادی تفکر داده شود بهتر می تواند با مشکلات خود کنار بیاید و در مورد آنها تفکر کند.
13- با مهر و محبت و تشویق و همینطور با تمرین بسیاری از مشکلات اولیه لکنت از بین می رود و در ضمن تمرین باید هیجانات و اضطراب کودک را به شکل های مختلف از بین برده و در حالت آرامش با او به تمرین بپردازید و باید دنبال سر منشاء ترس ها و اضطراب ها باشید ، علت آنها را تشخیص داده تا بتوانید آنها را از بین ببرید و در رشد سخن گویی آنها موثر باشید که این کار با کمک یک روانشناس کودک امکان پذیر میباشد.
14- والدین، مربیان و دوستان نباید هنگام تربیت یا بازی کودکان را دچار ترس کند چون ترس باعث اصلی لکنت زبان می باشد.
