– آیا تاکنون کاری به آن حد خطرناک که آسیبی به تو زده یا زندگی ات را به خطر انداخته باشد، کرده ای؟
– آیا تاکنون به کسی گفته ای که دوست داری بمیری یا به کشتن خودت می اندیشی؟
– فکر کن که مرده باشی در آن صورت چه اتفاقی خواهد افتاد؟ فکر میکنی مردن به چه چیزی شبیه است؟
– آیا خاطرت هست وقتی به خودکشی می اندیشیدی یا میکوشیدی خودت را نابود کنی، چه احساسی داشتی؟ 
– آیا تازگی ها اتفاقی افتاده است که تو با خانوادهات را نگران و مضطرب کرده باشد؟
– آیا تازگی ها هیچ مشکلی از نظر احساس غم و اندوه، بی خوابی، بی اشتهایی، زود عصبانی شدن یا خستگی همیشگی داشته ای؟
