اختلال دوقطبی

متخصصان بالینی، افرادی را که دوره های منیک دارند، حتی اگر هرگز دوره افسردگی نداشته باشند، مبتلا به اختلال دو قطبی تشخیص می دهند.
اصطلاحی که جایگزین «منیک دپرسیون » شده است.
برای اینکه متخصص بالینی فرد را مبتلا به اختلال دو قطبی تشخیص دهد:
او باید دوره ی منیک را به صورتی که در مجموعه ملاک های 5-DSM منظور شده است، تجربه کند.

ملاک های 5-DSM برای اختلال دوقطبی (دوره ی منیک)

1. دوره ی متمایز خلق که به طور نابهنجار و مداوم بالا، گشاد (بی تکلف) یا تحریک پذیر است .
 افزایش نابهنجار و مداوم فعالیت یا انرژی هدفمند که حداقل ۱ هفته ادامه می یابد.
 در بخش عمده ای از روز، تقریبا هر روز، وجود دارد (یا هر مدتی اگر بستری کردن ضرورت داشته باشد).
2. در طول دوره اختلال خلقی و افزایش انرژی با فعالیت، سه نشانه زیر (یا بیشتر) (چهار نشانه اگر خلق فقط تحریک پذیر باشد).
با درجه ی قابل ملاحظه ای وجود دارند و بیانگر انحراف محسوس از رفتار عادی هستند .
  1. عزت نفس کاذب یا بزرگ منشی.
  2. کاهش نیاز به خواب (مثلا بعد از فقط ۳ ساعت خواب احساس می کند استراحت کرده است).
  3. پرحرف تر از معمول یا فشار برای ادامه دادن صحبت.
  4. پرش افکار با تجربه ذهنی که افکار شتاب دارند.
  5. حواسپرتی .
  6. افرایش فعالیت هدفمند .
  7. مشغولیت بیش از حد در فعالیت هایی که توان زیادی برای پیامدهای ناراحت کننده دارند. 

3.اختلال خلقی به قدر کافی شدید هست که عملکرد اجتماعی یا شغلی را مختل کرده.

یا بستری شدن را ایجاب کند تا از صدمه رساندن به خود یا دیگران پیشگیری شود با ویژگی های روان پریشی وجود دارند.

4. این دوره ناشی از تأثیرات فیزیولوژیکی مواد (مثل سوء مصرف مواد مخدر، دارو، درمان دیگر) یا بیماری جسمانی دیگر نیست.

دو طبقه عمده اختلال دو قطبی، دو قطبی نوع I و دو قطبی نوع II هستند.

تشخیص اختلال دو قطبی نوع I ، روند بالینی را توصیف می کند.
 در آن، فرد یک یا تعداد بیشتری دوره منیک را با احتمال، هر چند نه لزوما، یک یا تعداد بیشتری دوره افسردگی اساسی، تجربه می کند.
در مقابل، تشخیص اختلال دو قطبی نوع II، به معنی آن است که فرد یک یا تعداد بیشتری دوره افسردگی و حداقل یک دوره هیپو منیک داشته است.
ملاک های دوره هیپو منیک، به صورتی که در مجموعه ملاک های زیر نشان داده شده اند.
پشبيه ملاک های دوره منیک هستند، اما کوتاه مدت تر هستند (به جای ۱ هفته، ۴ روز).

بیانگر انحراف محسوس از رفتار عادی هستند، و با درجه قابل ملاحظه ای وجود داشته اند:
  1. عزت نفس کاذب یا بزرگ منشی.
  2. کاهش نیاز به خواب (مثلا بعد از فقط ۳ ساعت خواب احساس می کند استراحت کرده است).
  3. پرحرف تر از معمول یا تحت فشار برای ادامه دادن صحبت.
  4. پرش افکار یا تجربه ی ذهنی که افکار شتاب دارند.
  5. حواسپرتی 
  6. افزایش فعالیت هدفمند یا سراسیمگی روانی – حرکتی.
  7. مشغولیت بیش از حد در فعالیتهایی که توان زیادی برای پیامدهای ناراحت کننده دارند. 

3. این دوره با تغییر واضح در عملکرد ارتباط دارد که برای فرد به هنگامی که دچار بیماری نیست، غیر عادی است.

4. اختلال در خلق و تغییر در عملکرد برای دیگران قابل مشاهده هستند.

5. این اختلال به قدر کافی شدید نیست که عملکرد اجتماعی یا شغلی را به طور محسوسی مختل کند یا نیازمند بستری کردن باشد.

اگر ویژگی های روان پریشی وجود داشته باشد، این دوره، بنا بر تعریف، منیک است.

6. این دوره ناشی از تأثیرات فیزیولوژیکی مواد نیست. (مثل سوء مصرف مواد مخدر)

مورد کوتاه: اختلال دوقطبی نوع I، اکنون دوره منیک

 با این حال، تقريبا ۶۰ درصد افراد مبتلا به اختلال دو قطبی در صورتی که تحت درمان مناسب قرار بگیرند می توانند بدون نشانه زندگی کنند
اختلال دو قطبی بیشتر از تمام اختلالات روانی دیگر، در افرادی روی می دهد که مشکلاتی در رابطه با سوء مصرف مواد نیز دارند.

شروع زودتر، نمرات افسردگی بالاتر، نمرات مانی بالاتر و ارزیابی کلی عملکرد پایین تر.
سابقه ی تناوب سريع در سال گذشته و مصرف داروهای ضدافسردگی نیز تناوب سريع را پیش بینی می کنند.

اختلال ادواری خو

نشانه های اختلال ادواری خو از نشانه های اختلال دو قطبی مزمن تر هستند و شدت کمتری دارند.
افراد مبتلا به این اختلال، ملاک های دوره هیپو منیک را چند بار ظرف مدت دست کم ۲ سال برآورده کرده اند .
دوره های متعدد نشانه های افسردگی نیز دارند، اما هرگز ملاک های دوره افسردگی اساسی را نداشته اند.
بزرگسالان، کودکان، یا نوجوانان در چارچوب زمانی خود، هر بار به مدت بیشتر از ۲ ماه، هرگز بدون این نشانه ها نبوده اند.
ملاک های 5-DSM برای اختلال ادواری خو
1. حداقل به مدت ۲ سال (در کودکان و نوجوانان حداقل ۱ سال) چند دوره همراه با نشانه های هیپومانیک وجود داشته است.
2. در طول ۲ سال فوق (۱ سال در کودکان و نوجوانان)، دوره های هیپومنیک و افسردگی حداقل نیمی از مواقع وجود داشته اند.
 فرد هر بار بیشتر از ۲ ماه بدون این نشانه ها نبوده است.
3. ملاک های دوره ی افسردگی اساسی، منیک ، یا هیپومنیک هرگز برآورده نشده اند.
4. نشانه ها در ملاک 1 با اختلال اسکيزوافکتيو، اسکیزوفرنی، اختلال اسکیزوفرنیفرم، اختلال هذیانی، یا طیف اسکیزوفرنی و اختلال روان پریشی مشخص یا نامشخص دیگر بهتر توجیه نمی شوند.
5. نشانه ها ناشی از تأثیرات فیزیولوژیکی مواد یا بیماری جسمانی دیگر  نیستند.
6. نشانه ها، ناراحتی یا اختلال قابل ملاحظه ی بالینی در عملکرد اجتماعی، شغلی، یا زمینه های مهم دیگر عملکرد ایجاد می کنند.

مورد کوتاه: اختلال ادواری خو

{ لاری، صندوقدار بانک ۳۲ ساله است که برای تغییرات خلقی خود که به دوران ۲۶ سالگی او بر می گردند، جویای درمان است.
به مدت چند سال، همکاران، خانواده و دوستان وی بارها به او گفته اند که خیلی دمدمی است.
